Our two Complex Health Needs Teams (Community Teams) covering the east have recently merged and will now be known as Community Team East.
For most families who use these services, you will continue to see the same clinician at the same place, and there will be no change to appointments that you may currently have booked in. There will be a small number of people who use our service whose clinician will change; these families will be notified separately in the coming weeks.
The only thing that will change is the way you contact us. As a result of these changes, please be aware the numbers and email addresses you may have previously used may have changed.
